tadalafil is a medicine that is used primarily to treat erectile dysfunction (ED) in men and, in some cases, for pulmonary arterial hypertension (HTPA).
1. Action and mechanism for action
Action in DE:
-Tadalafil inhibits the enzyme phosphodiesterase-5 (PDE-5). This enzyme is responsible for degrading nitrogen monoxide (no)-cyclic monophosphate guanosin dependent (cGMP) in penile tissues. By inhibiting PDE-5, tadalafil allows cGMP levels to remain high. This causes a relaxation of smooth muscles in the penis, which increases blood flow to the penis and facilitates obtaining and maintaining an erection.
Action at HTPA:
-in PTA, PDE inhibition-5 per tadalafil helps to relax pulmonary blood vessels, decreasing pulmonary blood pressure and improving cardiac function and exercise capacity in patients with this condition.
2. Dose and administration
-the 5 milligrams dose is one of the available doses of tadalafil.
For DE:
-it can usually be taken as needed, with an effect that can last up to 36 hours (so it is sometimes called the "weekend pill"). However, the dose recommended by the doctor should not be exceeded.
For HTPA:
-the administration is regular, it's not a take as needed as in the DE. The doctor will determine the appropriate frequency of administration depending on the severity of the ATP and the patient's response.
3. Precautions and side effects
Precautions:
